New Employee Forms
Welcome to Lakeland! As a newly hired employee, you are required to submit employment forms upon hire, preferably one full week prior to your start date.
Please click on the links below to open or download the forms. Most forms are PDF writable and can be filled in online, then printed for your original signature, or saved to your computer or flash-drive to be printed at a later time. Click here for a printable checklist of required forms.
Please send an email to the Director of Human Resources, to schedule a time to bring your completed forms and appropriate forms of identification to Human Resources. Do not send completed forms to Human Resources via email, unless requested to do so.

Required Transcript Information
If completion of a college degree is a required/preferred qualification for your position (all faculty, administrative, and supervisory/professional positions as well as many staff* positions) and you did not previously submit official transcripts, please contact your degree granting institution to have official transcripts sent to the Human Resources Department (be sure they specify the department), c/o Lakeland Community College, 7700 Clocktower Drive, Kirtland, OH 44094. If transcripts are not received, you may not be eligible to continue in the current assignment or be offered future assignments.
