1. Download, print & complete all of the Campus Kids enrollment forms shown below.
2. Stop by Campus Kids in the L-Building, room L-01 with your class schedule and completed forms.
3. You will be billed a $10.00 registration fee.

A spot cannot be guaranteed for your child unless all forms are completed
