This form authorizes Lakeland and the partnering institution to share academic transcripts and records with one another as deemed necessary for the purpose of academic advising, transcript evaluation and the awarding of degrees and or certificates pending completion of a partnership program through Lakeland's Holden University Center.

When should I complete the form?
Active Lakeland students who have completed a minimum of 15 credit hours or more in the last two years can complete the University Partnership Intent to Enroll form.

Where can I access the form?

• Access directly via myLakeland (Login required before being directed to the form)
• On-line by logging into myLakeland click on the "Student" tab
• A hard copy of the form is available in Lakeland's Counseling Center.

Partnership students, faculty and staff must complete a Partnership Services Form every semester they are active. This provides access to Lakeland Community College services and amenities such as emergency alert system, wireless internet access, printing and copying, Athletic and Fitness Center, Lakeland Library, child care and Laketran.
